US post office --Form 245????

Can someone tell me what this form is to do??

I believe or i am told by a seller on here that it is to find an item that has been sent to me in the UK 3rd November 06 and has not arrived..?

I thought the info was on the recept you get from Post Office when you send the item.
opps forgot the seller lost that!mmmmmmmmmm
